For this, you have to complete the "Assassin's Run" challenge, without missing one shot, and all the bolts hitting the enemies head. This is difficult because of the unpredictable AI. Personally, after I began trying to get all the achievements on dishonored I gave up after that dlc, even after I completed all of the other ones :( For me that was actually one of the easiest ones, you really just need to aim it so that it's very slightly in front of his head (where he is going to walk to that is) and then fire and hope you got the timing right.
